Once you have completed this form, your interest to staff on this workshop will be recognised and we will be in contact with you about the role(s) you requested.
Please be aware that this will not guarantee that you will be selected to staff on this workshop.

I acknowledge that the photographs and videos taken during this workshop will be stored on the UK HARP office drive and videos may appear in the final workshop video - which will be posted on YouTube and shared with the communities. Whole workshop group photos or team photos may appear on Facebook, the website or the newletter. Additionally, I give consent for the following:
